Class	Description
Class 1	School Related Groups: Associated with or sponsored by the school district Examples: Parent Groups, BARC, Educational Foundation
Class 2	Municipalities or public organizations serving our community Examples: Pop Warner, Booster Clubs, Towns/Villages, Senior Citizens, BACC, Scout Troops, Civic/Service Clubs, YMCA
Class 3	Resident Based Groups: (65% or more of participants must reside in the District Examples: Child Care Providers, Dance Studios, Corporate Groups
Class 4	Non-Resident Based Groups: Examples: Child Care Providers, School Districts, Corporate Groups, Dance Studios

A certificate of insurance in a minimal amount of $1,000,000 of Commercial General Liability and Umbrella Liability is required.

Ballston Spa Central School District, 70 Malta Avenue, Ballston Spa, NY 12020, must be listed on the submitted Certificate of Liability Insurance as an additional insured certificate holder.

Additional Automobile Liability and Workers Compensation and Employer Liability Insurance will be required if paid staff or vehicle operations are implied in the Facilities Request.
